Where To Get Care

  • Primary Care Provider

    “I need medical care soon and feel safe to wait.”
    Your doctor’s office is the best place to get treatment for conditions that are not immediately dangerous. Many physicians have same-day appointments available. Find a primary care provider.

  • Urgent Care

    “My doctor is not available today or it is after hours and I need medical care today.”
    Go to Urgent Care when your condition is not life-threatening but needs attention promptly. Urgent Care is a good option when your doctor is not available. Find an urgent care location.

    Go to the nearest Emergency Department or call 911 for life-threatening illnesses and injuries.

Personal Health Records

  • Information About Getting Your Medical Records

    You do NOT need to come to the hospital or clinic to get your records.

    You can email our Customer Service Team at his-customerservice@stillwater-medical.org. We will email you back within 2 business days.

    You can call (405) 742-5737 to discuss your needs with our HIS Customer Service Team and we will help determine the delivery method that works best with your needs.

    We will ask you for personal identifiers to confirm your identity.

    If you haven’t already done so, now is a great time to set up your Patient Portal! If you need help, call (405) 533-6010 and speak to our Patient Portal Specialist.

  • What is a Personal Health Record?

    A Personal Health Record (PHR) is a medical record created by you, the patient. You may have more than one physician, pharmacy, medication or health care provider. A Personal Health Record ensures you have one, complete record of all previous healthcare you have received and all care you currently are receiving. You collect, maintain, and update a record of all medications and treatments from all health providers. This allows your doctors to make the most informed decisions to accurately diagnose and treat you.

    A Personal Health Record can be paper-based, Internet-based, or PC-based, whichever is most convenient for you and your family. Your personal health record takes a personal commitment but can prove to be an invaluable tool for you, your family, and your care providers.

  • How To Obtain My Health Record?

    Your privacy is of the utmost importance to us. Trust that protecting your right to health information privacy is something Stillwater Medical Center takes very seriously. Your health information will not be used or shared unless you grant your permission by signing an authorization form.

    To obtain a patient health record from Stillwater Medical Center, please click here to download, print, and sign (or have the patient sign) the authorized form. Please return by fax to the Stillwater Medical Center Health Information Services Department at (405) 742-5739. If you are a legal guardian of the patient, please include your legal papers granting guardianship.

  • How to Obtain a Birth Certificate?

    Birth Certificate Information

    For a certified copy of your Birth Certificate, please contact the Vital Records Division of Oklahoma State Department of Health at (405) 271-4040 or visit their website.

    If you are a new parent needing to complete information on your baby’s birth certificate, please call Stillwater Medical Center’s Health Information Services Customer Service at (405) 742-5737.

Patient Concerns

  • Patient Representative

    You are an important part of the healthcare team. If you or your family feel you have experienced a sudden change or worsening in your condition, notify your nurse immediately for a rapid assessment. Please help us by bringing to our attention any safety concerns you observe. 

    If you have a concern about the quality or safety of the care you receive or the safety of the environment, please notify our Patient Representative at (405) 742-5676 or any member of our staff. 

    Your call will be treated with respect and consideration. No adverse action will be taken against any patient, employee, or staff member reporting safety or quality of care concerns.

  • DNV (Hospital Accreditation Body) Patient Grievance Options

    If you have a concern regarding the quality of care provided to you or someone else, please consider contacting the healthcare organization first and ask to speak with their “patient advocate.” It is the responsibility of a “patient advocate” to address grievances against their organizations.

    If you are not able to resolve or diffuse the issue with the organization’s patient advocate and want to take further action, .DNV provides five channels for submitting a hospital complaint:

Pharmacy

  • Is the Stillwater Medical Center Pharmacy open to the public?

    The Stillwater Medical Center Pharmacy is not open to the public, however, we recommend the following pharmacies:

    Please note that some of these pharmacies offer after-hours prescription filling for current customers. A service fee beyond the prescription charge may apply for current or new customers for after-hours prescription service. Please visit their websites for more information.

Patient Portal Questions

  • How do I enroll?

    First, your email address must already be a part of your SMC medical record and you will need your medical record number. If you don’t know your medical record number or you need to add your email to your record, call the Patient Portal Specialist at (405) 533-6010. You may also enroll in person by visiting Health Information Services Monday through Friday, 8am – 5pm.

  • Some of my information is wrong. How do I correct this?

    Any errors in personal information can be corrected by sending a change request through the portal. To correct errors in medical history information, please notify the Health Information Services department at (405) 742-5737 or his-customerservice@stillwater-medical.org.

  • How do I communicate my medical concerns?

    Do not use the Stillwater Medical Center Patient Portal to communicate medical concerns for any reason. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 9-1-1 OR YOUR PHYSICIAN’S OFFICE IMMEDIATELY.

  • How do I see my results?

    Test results and radiology reports can be found by clicking the Health Record button on the portal home page. On the Health Record page, click either the “results” or “reports” buttons on the right. NOTE: Please allow 24 hours for results to be available.

  • Can I also see my family member’s health records?

    Parents of children under the age of 18 may request and access a portal account for their children. Adults who act as another adult’s healthcare proxy may access the patient’s healthcare information once the patient has completed and submitted a Proxy Request form. Download the Proxy Request form here. If you have legal authority over an adult patient, you can request proxy access on the patient’s behalf. You will need legal documentation of your authority and must submit it in person at the Health Information Services Department at Stillwater Medical Center.

  • How can I update the email address in my medical record?

    The email address in your record is the address that will be used for sending activity, resetting your password, and other communications. If you need to update it, please contact the Health Information Services department at (405) 742-5737 or his-customerservice@stillwater-medical.org.

  • How do I get the complete set of my medical records?

    We have partnered with Sharecare to improve our flexibility for our patients and customers when requesting medical records. You do NOT need to come to the hospital or clinic, call, or email to get records. With this new relationship, you can request records online at your convenience using your computer or mobile phone.
